This checklist includes, but is not limited to the following: Safe Access and Workspace Is there safe access and adequate space for movement available for emergencies, work areas, walkways and passageways, with adequate lighting / visibility? Working at Heights / Scaffolds / Platforms. Are ladders, stairways, and elevated platforms properly located and functioning? Do Scaffolds and work platforms have a valid inspection tag at access point? Are floor and man-hole openings etc. properly protected? Are scaffolds robust, crossed braced, with ladder access to the work platform,? Are guard- rails and mid- rails provided, and fully boarded with Toe boards fitted? Falling Objects Is overhead protection provided for all areas of exposure? Are spoil materials and construction materials safely stored away from edges of excavations and scaffolds? Have unnecessary tools, equipment, etc.been cleared from the work area? Are work area housekeeping to a good standard? Confined Spaces Is a confined space permit to work operation in place? Is Gas tester and or attendant present and recording readings? Is Standby person in position to remove persons from confined space? Planning Work for Safety Has the Contractor coordinated with his Sub-contractors and other operators at site to prevent congestion and avoid hazards? Has the Contractor identified utilities and services? Has Contractor provided temporary flooring, safety nets, scaffolding, barriers, PPE where required? Are STARRT Cards, safety inspection, work permit, MSDS documents availablebefore work starts? Electricity Have all High voltage lines been identified with warning/prohibition signs? Have all high voltage lines been moved or de-energized, or barriers erected to prevent contact? Are all distribution boards earthed, power tools triple insulated, and secure? Are electrical cablings properly tied together, and in good condition? Are electrical tools and leads in good condition and tagged with date of inspection REV 2 (22.9.2012) Trenches and excavations Has the trench been appropriately benched/shored and inspected by competent person? Is there safe ladder or slope access into trench? Are spoils heaps at least 1.5 meters from edge of excavation? Are excavations enclosed by barriers on all sides? Is there adequate KAHRAMA or other authority requested arrangements in place? Is the work area free from ground water, is there aback up contingency water pump available locally? Working Near Live Traffic and Moving Equipment Is there a physical concrete barrier between the work zone and live traffic? Are there designated ingress/egress access points for plant/equipment movement? Is there a safe work zone for workers on foot (WOF) ,eg inspectors, to operate inside? Are there speed restrictions, and warning signs and lights in place? Welfare Facilities Are toilet facilities adequate? Is drinking water available? Are first aid boxes available on site? Site Material Storage /Material Handling Equipment Is the material handling space adequate and all items stored correctly? Are volatile and temperature sensitive and combustible materials adequately stored? Are adequate numbers and types of fire extinguishers available and tested? Are materials and waste items being stored / disposed in a tidy and correct way? Cranes & Lifting devices Are Crane inspection certificates and daily inspection checklists complete and available for inspection? Are certified Riggers present? Are Flagmen present? Is lifting equipment in good condition? (slings, hooks, chains, nylon webbings etc) Is lifting area barricaded and warning signs present? Heavy Plant and Trucks Are flashing warning lights working? Are daily plant inspections available for inspection and completed daily? Are reversing sirens working on trucks? Are trucks obeying site speed limit? Are Flagmen present with flags? Are the lifting Operations safe? Personal Protective Equipment (PPE) Are employees wearing the standard issue PPE items? Are employees provided with PPE as required for the work environment?
