A local civil engineering company was fined S$100,000 on 24 Aug 07 under the Workplace Safety and Health Act for failing to ensure a safe workplace, resulting in the death of one worker.



>> 事故简要描述

In the morning of 17 June 2006, the worker was standing inside the bucket of a cherry picker while spray painting a flyover.



The bucket was hit by an on-coming double-deck bus as it had encroached over the edge of the road. The collision caused the worker to fall from the bucket that was 4 metres above the ground.

He landed on the pedestrian pavement and died on the spot.

>> 事故原因调查

1. The company had conducted a Risk Assessment (RA) for the project. However, it failed to identify the hazards arising from spray painting works near the edge of the road.

2. There were no safety measures instituted to eliminate or reduce the risk of accidental collision between the cherry picker and the moving vehicles on the road.

3. There were no control measures to ensure that the cherry picker did not encroach accidentally into the road.

4. There were no warning signboards / lights, barricades or pylons put up at the scene of the accident to warn motorists of the works carried out in the vicinity.

The company was convicted for failing to identify the risks and institute safety measures for spray painting works at the workplace. >> 事故教训

1. Under the WSH (Risk Management) Regulations, comprehensive RA shall be conducted for all work processes to eliminate or control risk through detailed plans/procedures for work to be carried out. Arising from the RA, appropriate safe work procedures shall be developed and implemented. RA guidelines and other relevant references are available at the MOM Website and WSHAC Website.


2. Safety measures such as putting up warning signboards/lightings, barricades or pylons should be implemented at the vicinity to warn motorists of the painting work to prevent accidental collision between the boom lift and the vehicles traveling on the road. Aerial work platforms (AWPs) such as scissor lifts, and cherry pickers shall be inspected and certified safe for use by an MOM Authorised Examiner once every 6 months.

3. Safety belt or harness with two lanyards should be worn by the worker working at height. The lanyards should be securely anchored to a suitable anchorage point or static line; in this case the bucket of the cherry picker to prevent the worker from being ejected or pulled from the bucket. 4. The persons working on AWPs should be protected by physical barricades to prevent falls. Most platforms are also fitted with a safety interlock system to prevent accidental toppling.
